Capita are seeking a Qualified Social Worker for a role based within a Hospital Social Work Team to assist in enhancing our clients standard of care.

In the role as Social Worker you will be working with patients admitted to hospital along with their families and carers. You will be responsible for organising workloads which may include vulnerable clients or those with complex problems. Your main aim will be to assist clients maintain their independence and to live as normal a life as possible. To do this you will mobilise not only the personal resources of the clients through casework or group work, but also draw upon domiciliary, day and residential services or financial aid available in the department, through outside statutory and voluntary agencies and through relatives, neighbours and volunteers in the community.

To qualify for this role we are looking for someone with previous experience of working within a Hospital Social Work Team who can;
*Visit and interview clients, assess priorities and write reports.
*Accept responsibility for action in relation to the liberty or safety of clients in emergency situations.
Liaise with colleagues and other agencies.
